1. About This Application Form

This form has been prepared so that applications concerning the exercise of the rights granted to data subjects by Article 11 of Law No. 6698 on the Protection of Personal Data (the “Law”) can be assessed properly and in accordance with the Communiqué on the Principles and Procedures of Application to the Data Controller (the “Communiqué”). You may print this page using the button below and fill in the form by hand, or save it as a PDF. If you prefer, you may also submit your application through the online application form .

2. Application Methods

• In person: With documents establishing your identity and the completed, wet-signed form, to Esentepe Mah. Kelebek Sokak 2/225 Marmara Kule İş Merkezi Kartal/İstanbul.
• Via notary: To the same address, with the phrase “Kişisel Verilerin Korunması Kanunu Kapsamında Bilgi Talebi” (Request for Information under the Law on the Protection of Personal Data) written on the notification envelope.
• Via KEP: To [email protected], with the same phrase written in the subject line.
• Via e-mail: To [email protected], using a secure electronic signature, a mobile signature, or the e-mail address you have previously notified to our Company and which is registered in our systems.

3. Information About the Applicant

Pursuant to Article 5 of the Communiqué, the information above must be provided in full.

4. Subject of the Request

Please state your request under Article 11 of the Law clearly and comprehensibly, and attach the information and documents required by the nature of the request.

5. Conclusion of the Application

Pursuant to Article 13 of the Law, applications are concluded free of charge as soon as possible and within thirty days at the latest, depending on the nature of the request, and the response is communicated in writing or electronically. Where the process additionally entails a cost, the fee set out in the tariff determined by the Personal Data Protection Board (Kişisel Verileri Koruma Kurulu) may be charged. Should it not be possible to establish that the application was made by the data subject, or should additional information be required, our Company reserves the right to contact you through its registered contact details.

6. Declaration of the Applicant

In line with the requests stated above, I request that my application to your Company be assessed pursuant to Article 13 of the Law and that I be informed accordingly. I declare that the information and documents I have shared in this form are accurate, up to date and my own.
